My config file is set up correctly and alt+enter doesn't do anything. The game goes fullscreen for like a half a second when I first start it then goes to a windowed mode. It almost seems to behave like it actually is fullscreen but there's a border around the game like it's in windowed. It's bizarre and it happens only with Bethesda games.

Yup, there are replacer mods out there like the ones King just posted, but to my dismay I've only found one replacer that properly replaced the dragon shouts with lighter-voiced versions instead of replacing them with Japanese gobbledygook or nondescript grunts, and it was never updated for the DLC shouts.
